• Any previous Identity card or a copy of the identity card.
• For UAE nationals: original passport and family book. And Personal Photo with white background
• For GCC nationals: an original passport, an original valid Gulf ID, a Personal Photo with white background, and proof of residence in the country.
• Resident/expat: original, valid passport with copy of residence visa (softcopy), Personal Photo with white background.

If the applicant is less than 15 years old also needs to submit a personal photo with a white background, an Original Passport and Soft copy of the Residency visa, a passport of the parent, or Emirates ID, and the original birth certificate. After making all the documents ready, you can apply for a replacement.
